Form 8655 is used to authorize a reporting agent to: Sign and file certain returns; Make deposits and payments for certain returns; Receive duplicate copies of tax information, notices, and other
written and/or electronic communication regarding any authority granted; and
Provide IRS with information to aid in penalty relief determinations related to the authority granted on Form 8655.

Taxpayers who wish to assign a Reporting Agent (designee) to electronically file and pay on their behalf for the forms listed below must complete Form IL-8655 and submit it to the designee named. The Reporting Agent must retain this form subject to inspection by the Illinois Department of Revenue (IDOR) and the Illinois Department of Employment Security (IDES).
